Johnny burns his right hand.He had to write with his left hand. He wrote real sloppy with his right hand. He have to hide his right hand in his pocket. It was tough for him to get a job. He touch Cilla and she was afraid of his hand. He said he had never had someone talk to him like that. It would be harder for him to be a silversmith. He had to go hungry

One long term effect of Johnny's injury is that he could no longer be a silversmith. Also he lost a place in the Lapham's home. Once he was practically kicked out of the house, it was hard for Johnny to get a job causing him to go hungry for most of the time like Nareth said.
